Back Pain and Menopause: Is Your Spine Crying or Just Judging You?

Let’s talk about that pain. Not heartbreak. Not the pain of realizing your favorite jeans are now “aspirational.” I’m talking about back pain—the dull, nagging ache that makes you question whether you slept funny or if your spine is just holding a grudge.
For many women, back pain creeps in during perimenopause and sticks around like a bad ex. And no, you’re not imagining it. Hormonal changes—particularly the drop in estrogen—can lead to inflammation, loss of bone density, and weakening muscles that support the spine. Add in poor posture, stress, sleep disturbances, and the joy that is middle-of-the-night mattress tossing, and you’ve got a recipe for spinal sass.
Why It Hurts More Now
Estrogen isn’t just about periods and hot flashes—it plays a role in bone and joint health. When levels drop, the small joints in the spine can become inflamed or unstable, causing stiffness and pain. Combine that with reduced core strength and hormonal fatigue, and your back is practically screaming for support.
Signs Your Back Pain May Be Hormone-Related
-
It worsens around your cycle (if you’re still cycling)
-
It flares with mood swings or sleep issues
-
It pairs with joint pain elsewhere
-
It improved with movement but worsens after rest
What You Can Do About It
-
Move your body—but gently. Daily walks, yoga, or stretching routines can build strength without strain.
-
Support your spine. Core exercises like pelvic tilts and bridges can stabilize and reduce pressure on your lower back.
-
Anti-inflammatory foods. Think omega-3s, leafy greens, and turmeric. (We see you, anti-inflammatory smoothie queens.)
-
Magnesium and vitamin D. Deficiencies are common in midlife and can affect muscle function and bone health.
-
Mind your stress. Cortisol spikes tighten muscles—especially around the shoulders and spine. Try breathwork or meditation to unwind.
When to Call in Backup
If your back pain is severe, radiating down your legs, or paired with numbness, get it checked. Menopause may be part of the picture, but it shouldn’t be the whole story. You deserve answers and relief.
